"Then it is updated a second time" This is misleading wording, because in this example, no environment is updated twice. There are two updates, but the second update happens to the environment that was created by the first update. Change to "Then this second environment is updated"
The fix for this bug does not appear in the Recommendation of 23 January 2007. It will be considered for a future publication (either an Errata document or some possible future version of the specification).
Actually, the suggested change *does* appear in the Recommendation.
