Vlad: indexes are easier to screw up than offsets, have to be careful here

Tl;dr Need to clarify what encoder and decoder should do wrt table directory ordering and CollectionFontEntry indexes, potential for confusion
Vlad: we should have a separate section with more detail on how this is encoded, and the decoder will fall out of that
Data type for table directory indices could be 255UInt16 instead of UInt16; saves N_fonts *N_table_entries bytes in most cases (50% of those bytes)
